Excused absence request form — Ask your election officials to fill this out and then give it to people at your school so you can be excused on election day.

School consent form — If your election officials require written consent from your school, have a teacher or administrator at your school complete this form.

Parent consent form — If your election officials require written consent from your parents, have them complete this form.

School flyer — Hang this flyer up at your school to recruit more people to be poll workers.

“Wear a mask” sign — Hang this sign up in the polling place where you’re working on Election Day as a helpful reminder to voters that they should wear a mask to protect themselves and others.
